Important School Documents and Forms

Birth and Baptismal Certificates

Please be sure your student has a Birth Certificate on file, as well as a Baptismal Certificate, if applicable.  Submit copies of these documents  to the Embers School Office.

Medical Forms

  • Medical (Required for K and all new students.)
  • Dental (Required for K and 2nd Grade students.)
  • Vision (Required for K and new students to IL.)

If your child has a food allergy, please fill out and return the Illinois Food Allergy Form.

Spirit Shirts

Embers Academy Spirit Shirts are now available for purchase from Dennis Uniform.

Spirit Shirts are worn on designated Spirit Days only. Please note that the red Embers Spirit Shirt will be the only Spirit wear shirt that will be worn beginning this 2021-2022 school year.

Summer Math and Reading

Summer Math and Reading Packets are posted on the Embers website under the Parents tab.
